We have filed our 1095-C and 1094-C through Dynamics GP, since required in 2015.Ā We recently received a letter, regarding 2015 and an employee who received subsidy, while he was covered on our plan.
In researching this, it was determined, that we were missing page 2 and 3 of the 1094-C forms, that GP generates.Ā I have looked at all the years, and there is no Part III to the 1094-C transmittal form created, which is on page 2, or Part VI on page 3.
Did we miss a step in generating this transmittal form?Ā (We file our forms, via paper, because we are under the required limit, but self-insured.)

?Hi Pamela, Attached is our documentation from 2015 when we first came out with some of the ACA information. If you go to the last page of this document, we have it noted that we do not populate Part 2 and 3 as that information is not stored in Microsoft Dynamics GP, let me know if you have further questions, thanks Terry

Thanks for the information.Ā Do you know if the blank forms, for page 2-3 will be added in the future?Ā Since there are parts of the transmittal form, page one, that are filled in manually, and not populated through the system?
